Outreach is hiring a Part-time Job Coach/Case Manager at the Employment Opportunity Center – Carol Stream to assess, coach, and serve the Employment Opportunity Center participants and ensure case management services are successfully provided to all participants, taking the lead on mental health consultation, and working with participants placed with Corporate Partners to ensure success.
Schedule: 25 to 28 hours a week, Monday – Friday.

Responsibilities:

• The PT Job Coach/Case Manager will carry a caseload of program participants who need support, instruction, and accountability to become successful employees.
• The PT Job Coach/Case Manager will conduct assessments to understand the needs of the EOC participants (work, job skills, basic, and academic needs) and support students in establishing goals and becoming successful.
• Partner with worksite supervisors and transport students when needed.
• The PT Job Coach/Case Manager will serve as an instructor/facilitator for the Soft Skills Training Class.
• Provide case management services and manage participants' assessments and intervention plans for basic needs, living situations, and mental or behavioral concerns.

Requirement

Qualifications:   

• Bachelor's degree in Social Work, Human Services, or a related field. MA/MSW is preferred.
• 3 to 5 years of direct service experience, including case management and work with youth or young adults.
• Demonstrated skill in assessment, engagement, and collaborative problem-solving with clients.
• Knowledge of public systems and resources.
• Strong organizational and communication skills.
• Sensitivity to the needs of individuals from low-income situations, high-risk families, and culturally diverse backgrounds.
• Proficiency in Microsoft Office.
• A valid driver's license and reliable transportation.
