For 50 years, Outreach has changed the lives of thousands of individuals, children, youth, and families in DuPage County, IL, and we are seeking to hire a diverse staff of highly competent professionals to pursue Outreach's mission in restoring hope and providing opportunities for people to become all that God intends them to be.

 

Outreach is hiring a Full-time Clinical Case Manager at Outreach Transitional Housing located between Wheaton and Carol Stream, IL, to strategically support the residents of Jubilee Village 2, Hawthorne House, and Host Homes as they journey towards their goals and self-sufficiency by providing case management as part of a collaborative team of professional staff and volunteers. 

 

Benefits & Compensation:

Responsibilities:

  • The Clinical Case Manager will manage a caseload of 8-10 therapeutic casework clients.
  • The Clinical Case Manager will work with clients to set goals related to employment, making and adhering to a budget, completing schooling through high school or beyond, seeking counseling for mental health needs, life skills, leadership development, and providing necessary transportation.
  • The Clinical Case Manager will advocate and connect residents to resources.
  • Support the Transitional Housing Programs by responding to intake calls, providing assessment services, conducting outreach, and making referrals for the homeless population.
  • The Clinical Case Manager will report to the Director of Transitional Housing.
  • Schedule: Monday – Friday, 10:00 am to 6:30 pm, including two to three evenings a month for life skills training at team meetings; other occasional evenings or weekends to meet client needs.

Requirement

Qualifications:   

  • MA/ MSW with at least 1 year of case management experience, or BA/BSW with 1-5 years of experience in case management. 
  • Demonstrated success as a case manager.
  • Experience working with marginalized populations and developing effective relationships with participants and outside service providers.
  • Passion for supporting young women and men to envision a successful future.
  • Resilient, active listener, self-starter, and initiator.
  • Ability to work independently, as well as collaboratively with a team.
  • Administrative experience and attention to detail.
  • Proficiency in Microsoft Office.
  • A valid driver's license and reliable transportation.
  • A commitment to Outreach's Statement of Mission & Faith.
